Where
FC Peoria fields located at the Midwest Sports Complex in Mossville (Peoria), Illinois.  FC Peoria has 20 premier fields in one location with ample parking available.

There will be a $5 weekend parking fee for the tournament per vehicle.  Parking passes will be available for purchase at the team registration.  You can pay for team passes at that time or families can purchase the parking passes upon arrival at the fields.
 
Who
Open to boys teams from U8 through U14 and girls teams from U8 through U18.  US Club Teams are welcome.  To view results from the Summer 2006 tournament click on Summer 2006 Tournament Scores.

Two Application Options:

2.  Print a copy of the blank On-Line Application and  fill in the blanks.  Mail the completed application and application fee to FC Peoria prior to or on Monday, October 8, 2007.

*  Please note all special requests on the application form.
An accurate email address on the application is very important as all communication will be by email.
*  Approximately 2 weeks before the tournament additional instructions about schedules and registration will be emailed to the person listed on the application. 
 

Mid-America Shootout Tournament Roster Click Here for the on-line Mid-America Shootout Tournament Roster.  This form must be submitted by Friday, October 19, 2007.  This roster information is used to prepare the scorecards for the referees.  Information submitted may be changed up until the tournament registration.
Roster Size
U8 and U9 up to 12 players
U10 and U11 up to 14 players
U12 and above up to 18 players
U8 and U9 teams will play 6 v 6 only
U10 and U11 teams will play 8 v 8 only
U12 teams will play 11 v 11 format
U13 teams and above will play 11 v 11 only
Teams may roster a maximum of 4 guest players
 
# of Games and Awards
4 matches for U8 through U11 guaranteed
A minimum of 3 matches for U12 and above
Participation awards will be given to each U8 through U10 player and a team trophy for the 1st and 2nd teams with highest points.  1st and 2nd place team and individual trophies for U11 and above.
1st Place teams in each U11 and older bracket will get a 50% discount to the Summer 2008 Mid-America Shootout
2nd Place teams in each U11 and older bracket will get a 25% discount to the Summer 2008 Mid-America Shootout

Registration Requirements & Forms
NO COPY MACHINE WILL BE AVAILABLE AT THE REGISTRATION SITE - YOU MUST BRING COPIES TO BE LEFT AT REGISTRATION.


1.  2007-08 Team Roster with Add/Drop Forms (if any) with the state's or league's approval stamp clearly visible.

2.  Player Passes - Each player must have a current/valid USYSA Player Pass or US Club Soccer Pass.

3.  Illinois Medical Release Form - Each player must have a completed Illinois Medical Release Form.  The Medical Release form does not need to be notarized and copies of insurance cards do not need to be attached.  (Click here for an Illinois Medical Release Form)

4.  Illinois Youth Soccer Association Sanctioned Tournament Roster - This is required by IYSA.  The original Tournament Roster must be submitted at registration by Illinois and out-of-state teams.  The Tournament Roster submitted at registration must be signed by the coach.  Player signatures are NOT required even if you are using an earlier IYSA form that has signature lines.   (Click here for the IYSA Tournament Roster)

5.  Guest Player Permit (if any).  Illinois teams must submit an Illinois Guest Player Form.  Out-of State teams must use their state guest player forms and have all necessary state approvals.  (Click here for an Illinois Guest Player Permit)

6.  Travel Permit.  Non-Illinois teams must have a travel permit approved by its state association to submit at Registration.

7.  2007 Tournament Team Agreement.  This form and all required attachments is required ONLY for US Club Soccer teams. (Click here for a 2006 Tournament Team Agreement Form NT)

8.  Mid-America Shootout Tournament  Roster.  Click Here for the on-line Mid-America Shootout Tournament Roster to submit by Friday, October 19, 2007.

9.  
Parking Fee.  If desired, you can prepay for weekend parking passes for the team.

A copy of the state approved Team Roster, add/drop forms, and Illinois Medical Release Forms for each player and the original IYSA Sanction Tournament Roster (and Travel Permit for non-Illinois teams) must be left with Tournament officials at Registration.  Completed forms and copies may be sent to the mailing address below prior to the tournament.

Referees
FC Peoria will provide 1 referee per field per game for U8 & U9 teams and 3 referees per field per game for all other age groups.  If you are a licensed referee and are interested in being a referee at the tournament please contact Randy Crady, Referee Assignor, 309.383.4604 or cradyrt@mtco.comOut-of-town referees working the entire weekend should contact Scott Huffman, Tournament Director directly at (309) 635-4093 to obtain complimentary rooms.
